SFOT

SFOT (Surgically Facilitated Orthodontic Therapy)

What is SFOT?
Think of your teeth like a house, and your jawbone and gums like the foundation. Before moving teeth, the foundation needs to be strong.

SFOT strengthens your bone and gums, helping your teeth move faster, safer, and more efficiently during orthodontic treatment (braces or clear aligners=Invisalign).

How your care team works together

Orthodontist/Dentist: Plans and straightens your teeth
Periodontist (Dr. Sperling/Dr. Yu): Strengthens and supports your bone and gums

Together, we coordinate your care for the best results.

When is SFOT recommended?
• Thin or weak bone
• Gum recession
• Crowded or difficult-to-move teeth
• Previous orthodontic relapse
• Concerns about long-term stability
• A desire to shorten treatment time

Advantages of SFOT
• Faster treatment (often significantly reduced time)
• Safer tooth movement
• Stronger bone and healthier gums
• More stable, long-lasting results
• Expanded treatment possibilities
• One-time procedure with lasting benefits

How does it work?

Phase #1: SFOT- a surgical procedure is done to stimulate your body’s natural healing response and bone and or gum grafting are added to strengthen support.

Phase #2: Orthodontic treatment (Braces or Clear aligners) will start after initial healing.

What to expect after the procedure (first few days)
• Mild to moderate soreness
• Swelling
• Minor bleeding
• Temporary difficulty chewing

Most patients return to normal activities within a few days.

After recovery

You can expect:
• Faster tooth movement
• Smoother treatment progress
• Stronger support for your teeth
• More stable final results

You’ll continue orthodontic treatment, often with quicker progress and fewer delays.

The goal
A smile that is not just straighter—but healthier, stronger, and built to last.

Why Collaboration Matters in SFOT

SFOT works best when your periodontist and orthodontist work together. The periodontist ensures your gums and supporting bone are healthy and makes the precise bone adjustments needed, while the orthodontist carefully moves your teeth. This teamwork helps your teeth shift safely, protects your gums, and improves the overall results of your treatment.
